Data Sharing Across Health & Human Services to Improve the Culture of Well-Being
Monday, May 23 from 3:45 p.m. – 5:00 p.m.

The recognition that health can no longer be considered in isolation from both its broader family and community context or from the human service programs that often do or could support and strengthen health outcomes continues to grow. This panel will include representatives from CMS discuss current drivers, like health care reform and HITECH, and other opportunities to share data across health and human services programs, as well as share insights gained from current on-the-ground examples from the private and public sector. Hear from 2-1-1 San Diego about how they provide a venue for social service and healthcare organizations to share information while viewing an opted-in client record within a native case management system. Panelists will also discuss recent work coming out of Hennepin County, MN and the ways in which they are addressing legal barriers to data sharing, with an eye toward coordinating services. Participants will also hear from Illinois’ Public Health Institute on how they are partnering across sectors to establish community-based collaborations for sharing data and information to improve health and human services at the community level.
